How Distraction Osteogenesis Works
The biological process that makes bone lengthening possible
Discovered by Dr. Gavriil Ilizarov in the 1950s, distraction osteogenesis harnesses the body's natural fracture-healing response. When bone is carefully cut and gradually pulled apart, new bone continuously forms in the widening gap.

A low-energy osteotomy divides the bone while carefully preserving the periosteum and blood supply — the tissues that will generate new bone.
Analogous to panel (a): hematoma forms at the cut site, initiating the healing cascade

A rest period may be prescribed before distraction. Its duration depends on the indication, anatomy, age, fixation method, and the treating team's protocol.

Active lengthening
Bone segments are gradually separated using a prescribed schedule. The team may change or pause that schedule in response to imaging, symptoms, alignment, and joint motion.

Until clinical and imaging criteria are met

Distraction stops and the regenerate gradually mineralizes. Loading is advanced only under the treating team's device-specific instructions and assessment of healing.

The rate is a clinical variable: distraction that is too rapid or too slow can create different problems. The treating team interprets imaging together with pain, neurologic symptoms, alignment, and range-of-motion trends before changing the schedule.
