Is the Femur or Tibia Safer to Lengthen?
Neither segment is universally safer. The femur and tibia expose different joints, nerves, muscles, and alignment problems, and the evidence is not a randomized head-to-head comparison of otherwise identical cosmetic patients. Segment choice should follow individual anatomy, the proposed method, and the treating team's documented experience. [1][2]
Different Anatomy, Different Failure Modes
Femur
Femoral lengthening can affect hip and knee motion, thigh-muscle tension, alignment, and the entry pathway for an intramedullary nail. Antegrade and retrograde nail approaches create different surgical considerations. Published cohorts should not be reduced to a claim that every femoral protocol heals faster or has fewer nerve problems.
Tibia
Tibial lengthening also requires management of the fibula and ankle mechanics. Equinus contracture, peroneal-nerve symptoms, compartment-related concerns, and coronal or sagittal alignment are recurring considerations in the literature. The exact risk depends on anatomy and technique.
What One Large Nail Cohort Found
A 2023 retrospective multicenter study reported an adjusted relative risk of 1.7 for any recorded complication in tibial versus femoral lengthening with FITBONE or PRECICE nails. The cohort included mixed indications, was 80% femoral, and used a specific complication classification. This is a useful signal, not a personalized prediction and not proof that the femur is always the correct cosmetic choice. [1]
Do Not Convert Typical Gains Into Safe Ceilings
Systematic reviews report achieved amounts across heterogeneous cohorts, but neither a population mean nor a device stroke defines a safe femoral or tibial target. Relative lengthening, pre-existing range of motion, alignment, nerve symptoms, regenerate quality, and therapy progress all influence whether distraction should continue. [2][3]
Lengthening Both Segments
Published pediatric and skeletal-dysplasia series describe simultaneous or staged femoral and tibial procedures, but those populations and external-fixation protocols are not direct evidence for elective adult multi-segment surgery. There is no universal minimum interval between segments. Recovery of the first segment, complications, implant status, function, and the next operation's demands must all be reassessed.

- Which joint and nerve findings in my examination most constrain this segment?
- What is your complication profile for this segment, device, and indication?
- How will alignment and the adjacent joints be measured during distraction?
- What findings would trigger a slower rate, a pause, or an earlier stop?
- How does the rehabilitation plan differ between femur and tibia at your center?
