What Limb Lengthening Surgery Does
Limb lengthening surgery divides a bone in a controlled osteotomy, stabilizes the segments, and gradually separates them so new bone can form in the gap. Muscles, nerves, blood vessels, skin, and joints must adapt at the same time. The procedure is therefore a prolonged treatment process, not a single operation followed by ordinary fracture healing. Reconstructive and Elective Uses
Reconstructive lengthening may address limb-length discrepancy, deformity, nonunion, bone loss, or skeletal dysplasia. Elective stature lengthening seeks height increase without correcting a functional discrepancy. The biology overlaps, but the indication, baseline health, acceptable trade-offs, devices, and evidence base can differ substantially. Outcomes from one population should not automatically be applied to another. [2][3]
The Main Phases
1. Operation
The surgeon performs the osteotomy and applies an external fixator, an internal lengthening nail, or a combined fixation strategy. The exact incisions, entry approach, locking screws, and associated soft-tissue procedures depend on the bone and method.
2. Latency
A protocol-specific waiting period allows the early healing response to begin before distraction. Its duration is prescribed by the treating team and varies with age, bone, indication, and technique.
3. Distraction
The device is adjusted in small increments according to a written schedule. Serial imaging, joint motion, nerve function, pain pattern, alignment, wound or pin sites, and device performance are monitored. The rate or goal may be changed, paused, or stopped.
4. Consolidation
After distraction stops, the regenerate matures while fixation remains. There is no universal rule that consolidation lasts exactly twice as long as distraction. Loading and device removal depend on radiographic and clinical progression, the implant, and the protocol.
5. Rehabilitation and Later Procedures
Physical therapy and safe mobility begin early and continue through distraction and consolidation. Return to work, independent walking, sport, and implant removal are separate milestones. For the titanium PRECICE IMLL system, current U.S. FDA instructions state that the device should be removed after one year. [4]

Why Monitoring Is Central
Potential problems include joint contracture, nerve symptoms, infection, poor or premature regenerate, malalignment, fracture, implant problems, blood clots, and unplanned surgery. Published rates vary because studies use different populations, devices, definitions, denominators, and follow-up. A percentage is meaningful only when those details are visible. [2][3]

- It is not made low-risk by using an internal nail.
